How do you know when it’s the right time to explore aged care homes?
The answer often comes down to one key question: Have your needs changed in a way that affects your ability to live safely and independently at home?
If you or a loved one are finding it harder to manage daily tasks, have experienced recent health changes, or feel that a higher level of ongoing care is needed, then now may be the right time to look into aged care homes — and begin the ACAT assessment process.
This guide explains what an ACAT assessment involves, how to prepare, and how to take the next step toward finding the right care.
What Is an ACAT Assessment?
An ACAT Assessment (Aged Care Assessment Team) is required to access government-subsidised aged care homes in Australia. It helps determine whether you're eligible for residential care and outlines the level of support you may need.
To get started, you’ll first need to register with My Aged Care by:
-
Calling 1800 200 422, or
-
Applying online at myagedcare.gov.au
During the call, you’ll be asked a series of questions to better understand your current health, living situation, and care needs. Make sure you have at least 10–15 minutes and your Medicare card handy.
Based on your responses, you’ll be referred for a comprehensive assessment with an ACAT assessor.
What to Expect During Your ACAT Assessment
An ACAT assessor — usually a nurse, social worker, or health professional — will meet with you in person, typically at your home or in a hospital setting. The assessment takes around 45 to 75 minutes .
Here’s what happens:
-
Consent & Comfort Check
Your assessor will explain the purpose of the visit and confirm that you’re comfortable proceeding. They may also ask for permission to speak with your carer, family members, or healthcare providers if needed. -
Application for Care Form
If you're applying for entry into an aged care home, you’ll be asked to complete a simple Application for Care form. -
Understanding Your Needs
The assessor will ask about your:
-
Current health and mobility
-
Memory and mental wellbeing
-
Support network (family, carers, services)
-
Day-to-day tasks you find challenging
-
Medications you’re taking
-
Social activities and lifestyle preferences
Be open and honest — the more accurate the information, the better your support plan will reflect your true needs.
-
Support Plan Development
After discussing your situation, the assessor will develop a Support Plan that includes:
-
Your goals and challenges
-
The level of care you are eligible for
-
Your preferences for aged care services
You’ll also receive a decision letter and a referral code, which you’ll need when applying to aged care homes.
How to Prepare for Your ACAT Assessment
To make the process smooth and stress-free, prepare the following:
-
Your Medicare card and another form of ID
-
Contact details for your GP or specialist
-
Information about any support you currently receive
-
A list of any questions or concerns you’d like to raise
-
Any documents or referrals you think are relevant
-
A support person , such as a family member, friend, or interpreter, if helpful
Questions to Ask During Your Assessment
You might want to ask your assessor:
-
What types of care am I eligible for?
-
What happens after the assessment?
-
How soon can I expect a place in an aged care home?
-
Are there culturally appropriate providers in my area?
-
Can my carer receive any support during this transition?
-
What do I do if my care needs change over time?
What Happens Next?
After your assessment, you’ll receive a formal approval letter. This outlines:
-
The types of care you are approved to access (e.g. aged care homes)
-
A referral code used to apply to aged care providers
-
Your personalised support plan
