What to expect with ACAT assessment

To access government-funded aged care services — including home support or a place in an aged care home — your loved one will first need to complete an ACAT assessment (Aged Care Assessment Team).
This is an important step, and we’ll walk you through exactly how it works.
Start by registering with My Aged Care , the national gateway for aged care services. You can do this in one of two ways:
Call 1800 200 422 (have your Medicare card handy)
Apply online at myagedcare.gov.au
During the call or online process, you’ll be asked a few questions about your current health, daily routines, and support needs. Make sure you have at least 10–15 minutes free to complete this step.
After registering, My Aged Care will refer you to an ACAT assessor. This is usually a qualified nurse, social worker or other healthcare professional who will work with you — often through a face-to-face meeting at your home or a hospital — to understand your situation.
They’ll help determine what level of care your loved one is eligible for, and what services could support their wellbeing and independence.
Following their ACAT assessment, they will receive a formal approval letter outlining:
What types of care they are eligible for (e.g. residential care, home care, respite)
A support plan based on their unique needs
Their unique referral code (required for applying to aged care homes)
You can then begin exploring and applying for aged care services that meet their preferences.
